Citizens who had their personal information compromised due to the January 2023 data breach involving Imagine360, LLC, may be eligible for a payment of up to $5,000 if they submit a claim within the next two months. For eligible claimants, the window to take action is closing soon. A class action lawsuit settlement agreement has been reached, and all eligible individuals will need to see if they qualify and submit a claim by July 31, 2025, in order to receive compensation or other benefits.

What transpired during the Imagine360 data breach?

Imagine360 is a company that provides services including that of health plan administration and telemedicine. Around January 30, 2023, Imagine360 experienced quite a significant data breach where hackers were able to gain unauthorized access to sensitive information. Hackers had access to sensitive personal data such as Social Security numbers, medical records, and health insurance information of the individuals who used Imagine360’s services through their employer-sponsored health plans.

As such, the company was accused in a class action lawsuit (Collins v. Imagine360 LLC) of not sufficiently applying cybersecurity measures to prevent this data breach. While Imagine360 has not admitted to any wrongdoing, it has agreed to a $475,000 settlement to resolve these allegations.

What could be received from this settlement?

Al those citizens who received a breach notification from Imagine360 are part of the Settlement Class. These eligible members have been notified that their information was compromised, and these eligible citizens are expected to file a claim for the settlement.

Citizens who filed a claim could qualify for up to $5,000 for documented losses. The amount received from this settlement is similar to that received from the Ingo Money data breach settlement. Citizens can receive reimbursement for-out-of pocket expenses directly linked to the breach. Proof of out-of-pocket expenses include:

  • Bank fees
  • Phone and mailing costs
  • Travel expenses related to handling issues related to the breach
  • Credit monitoring services that citizens had to purchase
  • Time spent handling issues pertaining to identity theft

Citizens who do not have documentation of loses due to the data breach can consider the option of a flat payment of approximately $75 when a valid claim has been submitted. Like with all settlement agreements, the final payment may be adjusted depending on how many valid claims are submitted.

Ways to file a claim

For this particular settlement, the deadline to file a claim is July 31, 2025. All claims can be submitted by mailing the completed form to the settlement administration or via online via the website.

While eligible claimants have the option of claiming, they can also:

  • Object to the settlement by submitting all objections by July 16, 2025, should you disagree with any part of the terms but still wish to be included in the settlement.
  • Exclude yourself from the class action settlement by July 16, 2025, to retain your right to sue Imagine360 separately.
  • Not act and receive no settlement benefits and lose any right to sue Imagine 360 separately.

The final court hearing for the approval of the settlement is August 15, 2025.

Check qualification status now

There are only two months left to file a claim, so action is warranted. Citizens who received a notification from Imagine360 about the breach will certainly qualify for this settlement and may be entitled to a cash payout, free credit monitoring, or both. Since all claims are submitted under penalty of perjury, accuracy and honesty is of utmost importance and will surely pay off.

Citizens who are still uncertain as to whether they qualify should visit the FAQ section on the settlement website or contact the settlement administrator immediately. This settlement can make citizens richer if they can provide documented evidence that the data breach compromised personal information and had a devastating effect in the same way as you can become rich from the $25,000 T-Mobile settlement agreement.
